Form Submit - formaction

Hi there,

I'm currently trying to get a Submit Form to work correctly but am having issues with how I want the information posted.

On one page I will have the user fill out a name, title, date, piece of writing.

Once they have filled out the required Information I want the name and title to appear on another page that acts as a link to the writing.

I believe I have the form set up correctly but not sure how to get the functionality of the formaction.

I'm pretty inexperienced with this and am learning as I go,
