Search by Criteria and Preview image in JavaScript

Hello,

I need help to figure how to search with 2 search criteria: image number and date to upload and preview image. All I need is to figure how to index those 2 search criteria when I click in the upload button to populate the image. Any help will be appreciate it. zip file Attached.
